Position SummarySubstance Use Disorder (SUD) Case Manager is a member of the Behavioral Health team and provides a full range of case management services for patients diagnosed with a substance use disorder (SUD). This role is responsible for identifying, referring and authorizing appropriate services for patients assigned to them. The Case Manager is responsible for accessing, coordinating and ensuring the coordination of services and supports required for clients involved in substance use disorder treatment services.
Essential Duties and Responsibilities
- Provide advocacy to individuals (supported representatives and supports) regarding assessment, patient-centered service planning, service coordination, and transition planning.
- Conduct case management assessments to determine an individual’s needs and goals.
- Coordinates and provides access to available alcohol and other drug or ancillary services, such as housing, childcare, medical or financial services.
- Work closely with families, service providers, and other stakeholders to develop and implement individualized care plans.
- Attending weekly clinical meetings, supervision, and various other team meetings.
- Forming and maintaining relationships with community partners through outreach.
- Collaborate with internal teams and external partners to enhance service delivery and support.
- Presenting collateral contact information and case management plan(s) in Huddles and group supervision meetings.
- Compile, prepare, submit, and maintain accurate records, files, forms, statistics, and additional information in accordance with agency policies, licensing and/or contract requirements.
- Maintaining timely case documentation.
- Other duties as assigned.
· Minimum two (2) years of experience in substance use disorder treatment, behavioral health, or primary care case management.
· Associates Degree in Human Services is required. Bachelors preferred.
· CADC or CRM preferred.
· Highly punctual and dependable.
· High level of accuracy and attention to detail.
· Ability to maintain discretion with sensitive and/or confidential information.
· Intermediate or higher proficiency in Microsoft Suite.
