Position Overview: The Care Coordination Specialist will support all Recovia locations by overseeing external referrals, ensuring clients receive case management and warm handoffs, and fostering relationships with key community partners. This role involves working closely with Recovia’s sales and marketing teams to strengthen existing referral networks and develop new partnerships. Additionally, the Care Coordination Specialist will leverage phone use and digital platforms to connect clients with clinical resources as well as vital social services such as food assistance, housing, and other essential supports.
Key Responsibilities (Subject to change):
· Referral Coordination:
o Manage clinical referrals to external providers, ensuring warm handoffs for seamless transitions in care.
o Track and evaluate referral results to ensure client satisfaction and effective service delivery.
o Document communications and services within patient charts.
· Social Service Support:
o Use digital platforms to connect clients with resources such as food, housing, transportation, and more.
o Identify and address barriers to accessing social services to improve client outcomes.
· Relationship Building:
o Collaborate with Recovia’s sales and marketing team to maintain and expand referral partnerships with healthcare providers, social service organizations, and other community stakeholders.
o Conduct outreach to foster new relationships with potential referral sources.
· Team Collaboration:
o Work closely with clinical teams, ensuring clients’ medical and social needs are addressed comprehensively.
o Serve as a liaison between Recovia, community resources, and clients to promote seamless care delivery.
Qualifications:
· Preferred: Bachelor’s degree in social work, healthcare administration, or related field.
· Or relevant experience in care coordination, case management, or healthcare/social services.
· Proficiency with Excel, referral management systems, and EMRs.
· Strong organizational skills, with the ability to handle multiple priorities effectively.
· Excellent verbal/phone and written communication skills, with a talent for relationship building.
· Ability to take the initiative to help develop functions of this new role!
