Position Overview
Under the supervision of the Social Work Manager, works collaboratively and effectively as a member of the primary service case management team conducting Case Management assessments. Using results of the assessments, develop and monitor Individual Service Plans with measurable goals that are responsive to the needs revealed by the assessments, and to client preferences; at the agency and in the community to help clients achieve the goals of the individual service plan; and providing effective services that respond to individual’s needs on their path to independence.
Essential Functions :
- Manage a caseload of clients with visual impairments by conducting Case Management Assessments and documenting information in the agency database and health record.
- Duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the services needed for the client to gain independence.
- Develop and implement individual service plan through agency, home or field visits.
- Work cooperatively with agency staff in developing and implementing client's service plan.
- Make referrals to Opportunities for Ohioans with Disabilities (OOD) with appropriate follow-up.
- Initiate and coordinate intra and inter Agency referrals, track and manage services.
- Follow-up on agreed upon service referrals, ensuring that clients' needs are being met in order to assist the client in reaching independence.
- Maintain daily case notes and enter information into database in a timely manner.
- Acts in accordance with federal and state rules, regulations, laws, and administrative rules pertaining to the provision of Social Work, Counseling and Case Management services, including those concerning the privacy and confidentiality of protected health information; reporting suspected abuse, neglect, and exploitation; and protecting the safety and health of clients and the community. Update all records to meet CARF standards.

The employee is occasionally required to use hands to finger, handle, feel objects, tools, or controls; and reach with hands and arms above shoulders. Must be able to lift bagged adaptive aids of up to 25 pounds. Must have the capacity to travel with or without clients; ability to go out in the community to make home, employer, and library, etc. visits to, with, or on behalf of clients.
Once hired, the employee must have reliable transportation. Employee / driver must have current and sufficient automobile insurance and an acceptable driving record according to CSC policies.
