Pay & Perks of Social Worker :
48 per hour
Good work / life balance : Only 40 hours per week, rotating weekends.
13 week assignments give you the flexibility to see if the company is a good fit.
Working on a Medical Unit
Possible temporary to permanent.
Requirements of Social Worker :
MD LBSW, LMSW or LCSW-C
Must have 1 years of social work and hospital experience in an acute care setting
Cerner and / or EPIC experience
13 week assignment, M-F, 8-5 pm, no weekends
Job Description :
Provides direct psychological, social and practical care to a select caseload of patients and families including social work assessments, problem identification, goal setting and implementation of care plans. Makes referrals to outside agencies and coordinates continuity of care. Works collaboratively with the multidisciplinary team to optimize patient outcomes. Will provide inpatient and outpatient therapy and discharge planning. Contributes to the attainment of organizational goals.
