POSITION: Service Facilitator
LOCATION: Mishomis Wellness Center
SUPERVISOR: Wraparound Care Lead Service Facilitator
SALARY: $18-23 per hour 40 hours/week plus benefits
THIS IS A REGULAR FULL-TIME NON-EXEMPT POSITION
JOB SUMMARY: The main responsibility of this position is to provide assessment and facilitation services for individuals facing complex challenges such as mental health issues or substance use disorders. A collaborative team-based approach is crucial for effective service delivery in this role. The service facilitator will oversee the entire assessment process, ensuring that the Service plan and its implementation are well integrated, coordinated, and monitored to help each consumer achieve their goals. This position aims to empower consumers to attain the highest level of independent functioning possible. Furthermore, it involves building collaborative partnerships and understanding the range of services and support available within the tribal community and the Bay Area, all focused on promoting positive wellness outcomes for those being served.
DUTIES AND RESPONSIBILITIES:
1. Responds to referrals / intakes and complete application process.
2. Completes assessment and service plans are ongoing.
3. Builds rapport with participants by using a strength based, trauma informed, recovery oriented, and culturally informed approach.
4. Coordinates the development of Wrap Around Care (WAC) Teams with a focus on enhancing the involvement of natural team members and system partners.
5. Coordinates and facilitates WAC Team meetings and ensures that all team members are informed of and included in WAC meetings and follow up information.
6. Guides the WAC meetings through the plan development process using Wraparound Principles and the 7 grandfather teachings.
7. Documents the plan development process by recording and distributing minutes of WAC meetings, writing progress notes, and updated care plans, safety/crisis plans, and planning documentation under the WAC model that is guided by the Tiwahe framework and outcome measures.
8. Provides other case management functions as necessary, securing formal resources, and representing the WAC process in written documentation and verbal presentations to both internal and external partners.
9. Identifies temporary and permanent sustainable resources and services within family and community environments
10. Participates in the Internal Utilization Review process and other peer-review mechanisms, providing feedback to colleagues, accepting feedback from colleagues, and promptly responding to recommendations.
11. Serves as role model for the wraparound process, demonstrating a commitment to family safety, unconditional care, family voice and choice, team-driven decision-making, strength-based planning and interventions, flexible services and supports, and community partnership and collaboration.
12. Provides leadership in the overall assessment, development, implementation and evaluation of individualized wraparound plans
13. Prepares Individualized Family Plans and Safety Plans and coordinates each plan’s implementation, encouraging the maximum use of natural resources and community-based services, and supplementing these with flexible wraparound staffing and funding
14. Provides and completes timely documentation of all delivered services, including but not limited to assessment, treatment planning, youth and family skill building and education, and care coordination.
15. Maintain a professional and cooperative relationship with all programs involved with providing services to the tribal community and surrounding area.
16. Attends meetings, supervision and case management staff meetings as scheduled.
17. Maintain a confidential client record keeping system in accordance with program and state regulations and guidelines.
18. Participate in continuing educational opportunities, which promote personal and professional growth.

QUALIFICATIONS:
1. Bachelor’s degree in social work, psychology, or human services preferred. Associates degree required.
2. A minimum of two years of experience providing case management or service facilitation is preferred.
3. Experience working with Native American populations; experience maintaining extensive client file documentation
4. Meet or exceed the minimum standards of character as prescribed by P.L. 101-630 and WI State Statutes s. 48.685 and s. 50.065 and as subsequently amended and applied to certain classes of Tribal employees.
5. Must have a valid driver’s license, vehicle and appropriate vehicle coverage and maintain driver’s eligibility as a condition of employment. Be eligible to be put on the tribe’s vehicle insurance policy.
PERSONAL CONTACTS: Considerable daily contact will be made with the clients, general public, tribal leadership, program directors and staff, consultant(s), and the Tribal Community.
PHYSICAL REQUIREMENTS: The duties assigned to this position involve bending, stooping, lifting, and carrying. Items may be placed in overhead storage or shelving.
WORK ENVIRONMENT: All Tribal Buildings are smoke free. Able to travel throughout the community, make home visits, attend meetings, and participate in community educational programs. Private residences, when utilized for professional activities, will not necessarily meet the criteria of the established environmental safety regulations.
TRAVEL REQUIREMENTS: The ability to travel and attend meetings and trainings; overnight and out of town, both locally & nationally.
