Position Summary:
The Red Rock Center for Independence (RRCI) is seeking compassionate, detail-oriented, and dedicated Support Coordinator to provide case management and support coordination services to people with disabilities throughout Utah. This is a commission-based, independent contractor position serving clients on the Department of Services for People with Disabilities (DSPD) waivers. Coordinators ensure that services are person-centered, goal-oriented, and compliant with state and federal guidelines.
Key Responsibilities:- Provide comprehensive case management for individuals with disabilities on DSPD Medicaid waivers.
- Collaborate with individuals, families, providers, and interdisciplinary teams to develop and maintain Person-Centered Support Plans (PCSPs).
- Monitor and document service delivery, progress toward goals, and individual satisfaction.
- Conduct regular in-person and virtual visits as required by DSPD guidelines.
- Ensure services are delivered in accordance with the individual’s PCSP, waiver requirements, and state policies.
- Advocate for individuals' needs and coordinate access to community resources and supports.
- Maintain accurate and timely documentation, billing, and communication logs.
- Participate in required trainings and stay current on DSPD policies and procedures.
- Bachelor’s degree in Social Work, Psychology, Human Services, Special Education, or a related field.
- At least one year of experience in case management or working with people with disabilities preferred.
- Strong understanding of person-centered planning, Medicaid waiver programs, and DSPD services is a plus.
- Ability to manage a caseload of up to 40 individuals with varying levels of support needs.
- Self-motivated, organized, and able to work independently as a 1099 contractor.
- Excellent interpersonal, communication, and documentation skills.
- Must pass a background check and complete required DSPD certification and training.
- Reliable transportation and willingness to travel for in-person visits across assigned regions.
The position comes with a $7,000 signing bonus and is a commission-based independent contractor position. Compensation is based on the number of individuals supported and services delivered. Support Coordinators are responsible for their own taxes, insurance, and business expenses.
