QUALIFICATIONS:
· Previous experience in case management or behavioral health services preferred.
· Knowledge of mental health disorders, treatment modalities, and community resources.
· Strong interpersonal skills and ability to establish rapport with diverse populations.
· Excellent communication, problem-solving, and organizational skills.
· Ability to work independently and as part of a collaborative team.

- Energetic, self motivated, maintains professionalism and confidentiality.
- Good judgment, discretion and decision-making skills.
- Current computer skills necessary to complete job function.
- Current certification in CPR and First Aid.
- DPS Fingerprint Clearance.
· Valid driver's license and access to reliable transportation for home visits and community outreach.
DUTIES AND RESPONSIBILITIES:
· Assist the clinical staff with therapeutic services.
· Meets with client and or family, as scheduled, for relevant behavioral health services.
· May recommend additional support services for clients and families.
· Participates in staff orientation and training as required.
· Documents all services provided consistent with licensure and agency requirements.
· Coordinate with clinical team members to ensure continuity of care.
· Provide case management services, including advocacy, referral, and linkage to community resources and support services.
· Provide life skills services as directed by the clinical staff.
· Collaborate with external stakeholders, such as government agencies, schools, and employers, to advocate for clients' needs and promote their well-being.
· Participate in team meetings, case conferences, and trainings to enhance clinical knowledge and professional development.
· Serve as a liaison between clients, their families, and the treatment team to promote effective communication and engagement in treatment.
· Adhere to ethical and professional standards of practice, including maintaining confidentiality and respecting clients' rights and dignity.
