The Redding Rancheria is seeking a Recovery Case Manager. The preferred schedule for this position is Monday- Friday 8 am to 5 pm.
JOB SUMMARY:
The Recovery Case Manager will be part of the Recovery Program team and work directly with the Recovery patients and staff to effectively link patients to treatment options and other social services. The Case Manager will help patients navigating the healthcare system by providing case management, care coordination, outreach, support and linkage to care. The Case Manager may also provide health education services in the community to promote RRTHS services in Shasta and Trinity County, including assistance with the planning and operations of the syringe services program. The Case Manager will also be trained to assist as needed with other programs and services hosted by the recovery program. This is a versatile position that may require working in the field and with various departments within RRTHS.
REQUIREMENTS/MINIMUM QUALIFICATIONS:
- Must pass pre-employment drug screening.
- Bachelor’s Degree in social work, psychology, public health, or related field preferred.
- Documented healthcare experience required.
- Be in possession of valid and current California Driver’s License and reliable transportation.
- Must be proficient in using Microsoft Office Suite programs including Word, Excel, Publisher, and PowerPoint to make spreadsheets, graphs, manage data and create brochures, presentations and documents on computers as needed.
ESSENTIAL JOB DUTIES:
- Work as part of an interdisciplinary team to reduce stigma of addiction and support patient’s recovery.
- Assist in development and implementation of outreach strategies for new patients and/or community members currently using substances.
- Assists with purchasing and stocking supplies for the syringe services program.
- Provides case management services, referral and coordination with other allied professionals and treatment providers.
- Support and maintain strong relationships with community partners and promote RRTHS programs and services to the communities (Shasta and Trinity Counties).
- Initiates and coordinates individual care plans for clients, addresses both clinical and non-clinical components, and ensures the care plan is available to the patient, Recovery team, PCP, BH Therapist, psychiatrist, etc.
- Assists with the transition of recovery patients into treatment facilities and programs, including completing the necessary documentation, coordinating with the medical team, and scheduling transportation.
- Maintains accurate and timely documentation, records, and case files in NextGen.
- Develops and maintains knowledge of community based network of care to enhance wellness.
10. Maintains patient confidentiality and protects operations by respecting and maintaining the privacy and dignity of all patients.
11. Co-facilitation of Recovery support groups, as needed.
12. Administrative support, answering phones, and assisting patients with internal appointments
13. Maintains data, and monthly reports as required.
14. Other tasks as assigned by the Recovery Director.
