SUMMARY OF JOB:
Responsible for collaborating with HRHS, IWA, external referral sources and high quality partners to coordinate patient care with a focus on preventing readmissions and adverse health outcomes. Responsible for the provision of care coordination to assigned IWA patients, following them across the continuum of care (Healthcare, Homecare, Lifecare), with the goal of reducing barriers to efficiency and quality, while promoting clinical outcomes in accordance with relevant standards and benchmarks. The overall goal of this position is to enhance the quality of patient and physician management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of population health data analysis, case management, and utilization review and discharge planning.
The Population Health/Clinical Care Navigator (PHCCN) will serve as a care coordinator for clients who have been identified as ‘high risk’, ensuring timely and seamless coordination of services throughout the continuum of care, monitoring at-risk population through QA visits and assessments from admission to discharge. The PHCCN will understand and reinforce the importance of quality of life through chronic illness. Key performance indicators for this role will include reduced emergent situations and hospitalizations for their clients, timely and appropriate transitions to necessary services, including home health, private care and hospice and increased length of service for high risk clients. The PHCCN will participate in referral source readmission efforts including Bundled payment programs. Maintains excellence in standards of care and navigates timely, safe transitions of care.

Candidates must also demonstrate critical thinking and have excellent communication (both written and oral) skills. This role requires impeccable organizational skills and the ability to work and adjust to different environments. Must be willing to travel to HIGH QUALITY PARTNERS assigned practices, and have some flexibility in schedule. Must be comfortable with telephonic and face to face contact with patients, healthcare providers and community resources.
