SUMMARY OF JOB :
Social Services coordinator position identifies the needs of each resident / patient / families and assures that the needs are met by the appropriate discipline . Assists patients / residents / families in improving / maintaining and their ability to manage their physical, mental and psychosocial needs. Throughout their stay the social services coordinator supports the resident / patient's preference, choices and assists the families in coping with difficult situations.

RECRUITMENT REQUIREMENTS :
BSW or BA in sociology / social work ore related human services field
Minimum of one year experience in a long term care setting
Excellent oral and written communication skills
Knowledge of community resources
Experience working as part of an interdisciplinary team
Computer experience
