Redefining the Future Network, Inc. (RTFNetwork) is seeking a licensed Rehabilitation Specialist to supervise PRP workers and complete assessments and treatment plans for a growing PRP program. The Rehabilitation Specialist position is part-time with a minimum of 10 hours a week, which requires the development of clinical assessments and treatment plans for new clients authorized for services and reauthorizing clients that require continuation of services.
PROGRAM DESCRIPTION:
RTFNetwork’s Psychiatric Rehabilitation Program (PRP) assists individuals with mental illness to reach a higher level of independence. The PRP program assists participants with accessing and coordinating services and resources in their communities and works to ensure that everyone has access to the best possible treatment and care.
PRP services aim to maximize the community integration of our members and enable them to be successful and satisfied in the environment of their choice by working together to develop helpful rehabilitation goals.
Duties & Responsibilities
- Responsible for assessing and developing treatment plans for newly referred clients within five days of receipt of referral.
- Coordinate treatment team meetings 45 days before the expiration of services to reauthorize clients who qualify for the continuation of services.
- Participate in treatment team meetings.
- Collaborate with and clinically supervise assigned PRP Workers to discuss pertinent clinical information regarding the treatment plan and service delivery.
- Collaborate with therapists and treatment team members.
Experience
A licensed mental health professional (LCSW-C)
- Working with children and adolescents with a mental disability: 2 years (Required)
Minimum of two years of direct care experience working with youth with a severe emotional disorder.
Comfortable with using a computer and medical software.
Experienced in the mental health or human services field.
Able to manage a flexible but committed schedule.
Able to pass a criminal background check.
License/Certification:
- Driver's License (Required)
- Certified Psychiatric Rehabilitation Practitioner (CPRP)
- LCSW-C OR hold the Certified Psychiatric Rehabilitation Practitioner (CPRP) and/or Child and Family Resiliency Practitioner (CFRP) certification.
