What are the responsibilities and job description for the Care Manager Asst Non-RN position at RediMed/Business Health?
Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge.
Identifies and problem solves with other disciplines deficiencies that contribute to inappropriate uses of resources, less than desirable outcomes, delays in discharge and disruption to continuum of care.
Collaborates with patient, company (when necessary), carrier and medical providers regarding the anticipated plan of care while setting goals with them to promote responsibility for self-care.
Assists with occupational health and wellness onsite services.
Maintains departmental records and charts.
Performs chart reviews in order to actively manage resources and ensure supporting documentation adequately reflects patient status.
Maintains current knowledge of practice and skills regarding case management skills through education, literature review and presentation.
Participates in company tours to evaluate physical demands to perform jobs and identify ergonomic improvements to improve safety and decrease injury rates.
Coordinates with billing staff, when necessary, that all services are being charged for and then billed out to the correct company.
Communicates and reports the necessary results of occupational health and work comp service to the appropriate individuals (employer or employee) in a timely manner.
In conjunction with BH medical provider, manages on-going BBP exposures through tracking and communication to all involved.
Follows HIPAA guidelines when reporting results out to appropriate individuals. Assures appropriate documentation is obtained to release results.
