What You’ll Do as a Care Logistics Referral Coordinator

Responsibilities

You’ll play a key role in connecting patients with the care they need, supporting their journey to better health. Responsibilities include:

• Managing inbound and outbound referrals for specialty providers.
• Scheduling specialty services and maintaining regular communication with providers.
• Ensuring referral tasks are completed within 72 business hours.
• Documenting referral details in our EMR system.
• Answering patient inquiries with kindness and professionalism.
• Supporting team members as needed.
  
  

What We’re Looking For

• Proficiency with Microsoft Word, Excel, and EMR systems (preferred).
• Bilingual (Spanish/English) candidates are highly desired.
• High school diploma or GED required.
• Ability to pass a National Agency Check with Inquiries (NACI) background investigation.
  
  

What You’ll Earn

• Starting pay: $17.36/hour.
