What You’ll Do as a Care Logistics Referral Coordinator
Responsibilities
You’ll play a key role in connecting patients with the care they need, supporting their journey to better health. Responsibilities include:
- Managing inbound and outbound referrals for specialty providers.
- Scheduling specialty services and maintaining regular communication with providers.
- Ensuring referral tasks are completed within 72 business hours.
- Documenting referral details in our EMR system.
- Answering patient inquiries with kindness and professionalism.
- Supporting team members as needed.
- Proficiency with Microsoft Word, Excel, and EMR systems (preferred).
- Bilingual (Spanish/English) candidates are highly desired.
- High school diploma or GED required.
- Ability to pass a National Agency Check with Inquiries (NACI) background investigation.
- Starting pay: $17.36/hour.
