ReDiscover is looking for a Health Coach to add to our Health Care Home Department!

ReDiscover has been known for over 50 years as a non-profit community mental health center in southeastern Jackson County, Missouri with a mission of delivering efficient and cost-effective treatment for the people we serve. Our diverse team of over 700 employees is “Passionate About People” as we support our community, specifically those whose lives have been affected by a mental illness or substance use.
WHAT WE DO
Healthcare Home is a population health management program designed to champion healthy living changes and promote preventative care, health education, and healthy activities for clients, staff, as well as the community.
At ReDiscover, while every day is different, a typical day may include the following roles and responsibilities:
- Maintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.
- Deliver personalized instruction, coaching, and motivational support to individuals and groups, helping them achieve their health and fitness goals in both one-on-one and group settings.
- Facilitate educational group sessions on a variety of topics, including nutrition, tobacco cessation, fitness, and other physical activities aligned with clients' interests and goals.
- Provide oversight of the exercise program on site.
- Develop and maintain relationships telephonically, and in the community with contracted gyms and other community partners.
- Outreach to referred clients to join the Health Club.
- Collaboration with the mental health team to ensure consistent treatment.
- Monitoring, maintaining, and entering data related to the client(s) physical health.
We are focused on your development and will spend time helping you continue to grow as a Health Coach. However, here are some that you will need prior to starting with ReDiscover:
- Bachelor’s Degree in a Social Science, health and wellness, or related field from an Accredited College or University.
- Certified personal trainer, nutritionist or dietician preferred.
- Valid Missouri Driver’s license. Must meet guidelines of agency’s driving criteria (i.e. Proof of liability insurance) and maintain the criteria as per company policy at all times.
- Working knowledge of mental illness and substance use disorders.
- Excellent customer service skills with a recovery focus.
- Strong written, oral and interpersonal communication skills.
- Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as G-Suite.
