The Kansas City Assessment & Triage Center (KC-ATC) is a comprehensive crisis and stabilization program serving the needs of Kansas City’s at risk population. Employing a multidisciplinary staff, KC-ATC is a resource for Law Enforcement, Hospital Emergency Departments and Community Mental Health programs in the greater Kansas City area.
At ReDiscover, while every day is different, a typical day may include the following roles and responsibilities:
- Provides brief assessment, discharge planning, and will assist in coordination of follow-up services as appropriate for adults who are ReDiscover Kansas City Assessment and Triage Center (KC-ATC) clients.
- Ensures clients receive the services they need in order to improve their functioning in the community and in accordance with the standards of clinical practice established by the State of Missouri and the policies and procedures of ReDiscover.
- Assists the facility as directed or in the absence of the Team Leader or Program Manager in answering all incoming calls, taking referrals, greeting visitors/clients, completing triage/intake assessments, obtaining collateral information and medical/psychiatric histories on clients in crisis arriving to KC-ATC.
- Collect and maintain information about clients and document interactions within ReDiscover’s designated electronic medical record.
- Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.
- Availability to work a part-time schedule (24 hours per week) Monday 3:00pm-11:30pm, Tuesday 3:00pm-11:30pm, and Thursday 11:00pm-7:30am. Weekend shifts might be available.
- Master's Degree in a social science field.
- A valid automobile driver’s license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work.
- At least two (2) years experience in a direct service for discharge planning in a mental health setting preferred.
- Knowledge of the community resources as well as health and social service systems in the Kansas City metro area and skill in establishing working relationships with community partners preferred.
- Working knowledge of mental illness and substance use disorders.
- Excellent customer service skills with a recovery focus.
- Strong written, oral and interpersonal communication skills.
- Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as GSuite.
