WHAT WE DO
The Peer Services program is an expansion program dedicated to outreaching and connecting clients to services for mental illness and / or substance use disorders. Our Peer Services provide support to clients using personal experiences to build rapport and engage in services that are offered.
At ReDiscover, while every day is different, a typical day may include the following roles and responsibilities :
- Supports ReDiscover's Hospital Diversion program, where staff work to reduce the frequency of visits to hospital emergency rooms.
- Shares lived experience of resilience and recovery and models successful recovery behaviors for individuals with a mental health diagnoses and / or a substance use disorder.
- Links clients with support groups and other community resources, which may include connecting with health & wellness activities, finding and maintaining employment, or other activities that would support clients' recovery goals.
- Works within an interdisciplinary team to develop and modify individual client treatment plans.
- May participate and / or facilitate mutual support and education groups within specific ReDiscover programs.
- May assist with client transportation, which could include transporting individuals in personal vehicle.
- Utilizes non-traditional means to connect with clients, which may include street outreach.
- Safeguards Protected Health Information (written and electronically) and maintains confidentiality. This may include providing written documentation.
