The Medication Assisted Treatment Coordinator plays a pivotal role in guiding patients and their families through the transition from hospital discharge to home, ensuring seamless care follow-through and monitoring of the treatment plan. This position is integral to supporting medication-assisted treatment in acute care settings.
Key Responsibilities:
- Direct discharged patients to appropriate follow-up care and assist with scheduling appointments
- Facilitate transitions from the hospital to outpatient treatment, obtain referrals or authorizations
- Arrange transportation as needed
- Contact patients prior to appointments to ensure follow-through
- Document patient advocacy activities
- Develop relationships with Primary Care Physicians and Specialists for smooth transitions and expedited medical appointments
- Maintain knowledge of local healthcare resources and collaborate with them
- Coordinate care with home health agencies and durable medical equipment companies
- Work with pharmacies for medication coverage, prescription pickup/delivery/discounts
- Keep patients, families, and caregivers informed of care management
Requirements:
- High school diploma or GED required
- Experience in customer services required
- Healthcare experience required
- Caregiver experience preferred
- Experience working with individuals in hospitals or public health settings preferred
- Experience with underserved populations preferred
Certifications/Licenses:
- California Bridge Substance Use Navigator Training required
