Summary

Redmond Oasis Village Project is a nonprofit organization committed to providing shelter, support, and resources to individuals experiencing homelessness in Central Oregon. We believe everyone deserves a safe and dignified place to call home, and our shelter plays a crucial role in building a path towards home for unhoused people in our community.

The Case Manager works individually with Participants to develop and support goals and progress towards stable housing. Case Management includes administration of the HMIS Data System, administering assessments, coordinating care and developing a housing plan with all Participants. The Case Manager participates in all program required trainings and activities.

Essential Functions

1. Make initial contact with Participant to onboard them into the Oasis Village program and provide referrals and/or concrete support through one-on-one meetings.

2. Upon enrollment, creates a working paper and electronic file with all necessary documentation. Complies with state, funder or other requirements of Participant files.

3. Assist Participants in developing their individualized goal/housing plan. Helps to coordinate medical, dental and mental health care within the first 90 days of shelter. Develops and oversees a formal housing plan with Participants at minimum after their 90-day point at Oasis Village, or sooner than within the first 90 days if determined by the Participant.

4. Guide Participants to identify and address issues their own and challenges.

5. Help Participants find and utilize resources, agencies, and programs that will help them achieve their goals towards stable housing.

6. Administer all required assessments and screenings with Participants.

7. Enter and maintain data as required by assigned program, funding source. Oversee and enter data into HMIS in a timely manner. Complies with all regulations and requirements of HMIS, including requests for reporting externally.

8. Refer Participant to external services and resources when Participant needs are outside the scope of Case Manager’s professional training.

9. While making a minimum of one meeting per week , create and maintain accurate and timely contact notes and enter timely data in appropriate program databases per program regulations.

10. Collaborate with relevant agencies to assist Participants with meeting goals.

11. Accompany, facilitate, and/or attend Participants’ meetings with other agencies, and/or legal related agencies. Provide or brainstorm ideas regarding transportation, as needed.

12. Follow all program policies and procedures, including interactions with Participants and employees.

13. Participate in the Coordinated Entry Process. Screens and determines eligibility for Participants’ acceptance into Oasis Village. Manages the waiting list for Oasis Village. Respond to perspective Participants that enquire about services.

14. Communicate with the team at Oasis Village regularly regarding Participant progress and needs and enlists the Mentors to support Participant’s goals.

15. Prepare program educational materials, or other assigned tasks that benefit the program and Participants.

16. Attends required program and staff meetings, training and professional development.

17. Maintains confidentiality as required by HIPAA and Oasis Village’s policies and procedures.

18. Supports the team by covering Mentors as needed to serve Participants and the program.

19. Works with the team to help with facility cleanliness as needed.

20. Other duties as assigned.

GENERAL DUTIES AND RESPONSIBILITIES

1. Develop and maintain up-to-date knowledge and information on programs and community resources.

2. Obtain necessary and ongoing training for services being provided.

3. Other duties as assigned.

Desired Qualifications

(Education, Certification, and Licensure)

● Bachelor’s degree in psychology, Social Work, Sociology or related field. Equivalent of 3 years of experience in related field may be substituted. Certification in Evidence Based Model such as Community Health Worker desired.

Desired Experience

● At least 3 years of experience working with diverse communities and people who are unhoused, people who have experienced trauma, people with mental and physical health challenges or other factors that contribute to vulnerability.

Desired Knowledge, Skills, and Abilities

● Knowledge of alcohol and drug addiction and detoxification, mental health, and community resources and recovery programs.

● Ability to communicate clearly, concisely, and effectively, both orally and in writing.

● Ability to build relationships with Participants.

● Ability to work well with people, including co-workers, and community partners of diverse backgrounds, and culture.

● Ability to work independently and as a collaborative team member.

● Ability to function effectively and use good judgment in crisis situations.

● Must be able to operate usual office equipment such as laptop, telephone, and copy machine.

● Ability to operate a motor vehicle.

Supervisory Responsibilities

● This position does not have supervisory responsibilities.

Schedule

● 8 hour shift

● Monday-Saturday as scheduled. Evenings and weekends may at times be required for this role to accommodate Participant’s schedule.

Physical Requirements & Work Environment

● Occasionally exposed to physically and/or verbally aggressive Participants

● Stand 25% of the time; sit 50% of the time; walk 25% of the time

● Assess and observe objects at all distances

● At times the job requires stooping, kneeling, crouching, reaching, handling items, and feeling items

● Light physical effort is required by handling objects up to 20 pounds occasionally

● Exposed to outdoor environment 20% of the time

● Noise level is usually moderate

● Job tasks may involve exposure to blood, body fluids, or tissue, but employment may require unplanned exposure.

Pay: $25.00 - $27.00 per hour
