About the Opportunity:
Primarily responsible for the development and implementation of stimulating Resident Activity Programs encompassing physical, intellectual, social, emotional and spiritual interest for the various levels in the community, including those residents with either dementia or Alzheimer’s Disease. The resident Life Story is an instrumental part of the resident’s activity plan. By communicating clearly and promptly with residents, families, staff and management, the Lifestyles Coordinator is also responsible for identifying resident issues and is instrumental in addressing their resolution.
What you will do:
- Prepare, plan, schedule and review calendar of activities for level of care appropriateness within assigned department.
- Encourage residents to attend activities and build enthusiasm for programs.
- Plan and schedule trips and outside activities; arrange for transportation and supervision.
- Ensures all staff members are responsible for providing a level of participation for our residents.
- Creatively respond to resident requests for programs/activities. Design events to meet those interests when possible.
- Address resident issues with residents, families and other department managers if appropriate.
- Understand the Masterful Moment approach.
- Meet with new residents upon move in to introduce Activities/Wellness programs and discuss the Resident’s personal interests/programs and Life Story.
- Complete all initial forms for admission based on interview;
- Report resident concerns/grievances by communicating with appropriate departments to ensure a timely response/resolution
- Conduct Care Conferences upon Admission, quarterly, annually and as needed.
- Other duties as assigned by supervisor
