Title: Clinical Liaison (LPN/RN)

Status: Full Time

Shift: Monday through Friday, 8:00 am - 4:30 pm, On-call rotation

Location: Westmoreland County - This role has a limited territory and reasonable census expectations.

What will I do as an employee with Redstone?

As the Clinical Liaison (LPN/RN), supporting the Redstone mission will include the following responsibilities:

  • The Clinical Liaison reports directly to the Marketing Manager and is responsible for maintaining and nurturing existing relationships with referral sources utilizing our Home Health and Hospice Programs. Conducts clinical rounds with physicians and visits within the local hospital systems. They serve as the main point of contact for clients while understanding their needs, providing support, and ensuring client satisfaction. They promote the image and philosophy of Redstone@Home.
  • Referral Management:
    • Build and maintain relationships with referral sources.
    • Coordinate referrals from intake to admission.
    • Monitor client status for potential re-admissions.
    • Achieve individual sales/volume goals.
  • Quality and Compliance Management:
    • Collaborate with internal teams to address client concerns.
    • Support quality and regulatory compliance with partner facilities.
    • Maintain knowledge of funding and government regulations.
    • Utilize technology to plan and document marketing activities.
    • Participate in department and program planning meetings.
  • Customer Service:
    • Conduct regular meetings with referral sources.
    • Identify needs of community seniors and referral sources.
    • Coordinate education and program services.
    • Participate in networking activities and events.
  • Other Responsibilities:
    • Provide clinical staffing relief for Home Health.
    • Participate in marketing/sales activities.
    • Participate in on-call programs.
    • Perform other duties as assigned.

What do I need to qualify for this role with Redstone?

  • A valid LPN or RN license in the state of Pennsylvania.
  • Previous experience in home health care/hospice/community health.
  • Experience in sales and marketing preferred.
