Our Community Based Services Professional will work closely with members to understand their needs and preferences, and develop personalized plans to support their goals and aspirations.
You will be responsible for providing essential support and care, including assistance with daily living activities, socialization, and recreational activities.
The ideal candidate will be highly organized, empathetic, and have excellent communication skills, with a strong commitment to maintaining confidentiality and ensuring accurate documentation and reporting.
Key Responsibilities:
- Develops and implements personalized plans to support members' goals and aspirations.
- Provides assistance with daily living activities, including bathing, dressing, feeding, and toileting.
- Encourages members to participate in socialization and recreational activities.
- Provides emotional and psychological support to members.
Requirements:
- 18 years of age or older
- High School Diploma or GED
- Hold a valid Arizona Driver's License (this position requires driving)
- Have documentation of current registration and car insurance
- Clean 39-month motor vehicle report
- Ability to complete training programs and successfully pass associated tests
- Must pass a background check and fingerprint clearance
