Job Title: Social Service Director

Reports to: Executive Director
Job Overview:
We are seeking a compassionate, dedicated, and experienced Social Service Director to join our team at Redwood Care Solutions, an assisted living facility and home health provider that provides exceptional care to our residents. The Social Service Director will play a vital role in ensuring that the emotional, social, and psychological needs of our residents are met while promoting their overall well-being. The ideal candidate will possess strong leadership skills, empathy, and a commitment to providing high-quality care to senior residents.
Key Responsibilities:
- Resident Assessment & Care Planning:
- Conduct comprehensive social assessments of new residents and periodically reassess existing residents.
- Develop, implement, and update individual care plans, collaborating with residents, families, and interdisciplinary team members to ensure residents’ social, emotional, and psychological needs are met.
- Ensure that each resident’s preferences, values, and goals are considered in their care planning.
- Resident Advocacy:
- Act as an advocate for residents, ensuring their rights are upheld, and they have a voice in their care decisions.
- Address resident concerns, complaints, and grievances in a timely, respectful, and effective manner.
- Promote a positive and supportive environment for residents and their families.
- Staff Supervision & Training:
- Supervise and train social service staff, ensuring they are equipped to meet the needs of residents.
- Provide ongoing guidance, mentorship, and professional development for team members.
- Community Resources & Referrals:
- Identify, develop, and maintain relationships with community resources and services that support residents’ well-being (e.g., counseling services, support groups, legal aid).
- Assist residents and families in connecting with external resources and make appropriate referrals when needed.
- Documentation & Compliance:
- Maintain accurate, up-to-date resident records and care plans in compliance with facility policies, state regulations, and accreditation standards.
- Ensure all documentation and reporting related to social services is completed in a timely and organized manner.
- Programs & Activities:
- Coordinate social and recreational programs designed to enhance the social engagement, emotional well-being, and overall quality of life for residents.
- Encourage resident participation in facility activities and ensure programs are tailored to diverse interests and needs.
- Collaboration:
- Work closely with nursing, medical, and other staff to ensure holistic care for residents.
- Participate in care team meetings, ensuring social service-related concerns and recommendations are integrated into residents' care plans.
Qualifications:
- Experience in social work, preferably in long-term care, senior services, or assisted living.
- Knowledge of social service programs, local resources, and regulatory compliance standards.
- Excellent interpersonal and communication skills, with a strong ability to build relationships with residents, families, and staff.
- Compassionate, patient, and dedicated to advocating for residents’ well-being.
- Ability to work independently and as part of a multidisciplinary team.
- Strong organizational skills with attention to detail.
