What are the responsibilities and job description for the Case Manager position at Redwoods Rural Health Center?
Redwoods Rural Health Center is seeking a full-time Case Manager to provide integrated, supportive and enabling services to individuals and families. Case Managers collaborate with an interdisciplinary care team to ensure continuity of care and assist patients in accessing services and utilizing resources to support their medical, dental, behavioral health, and socioeconomic needs. Case Managers identify and address a variety of non-clinical social determinants of health (SDoH) to promote positive health outcomes and reduce health inequities. Case Managers are dedicated to serving marginalized or otherwise underserved populations with the lowest access to resources in our community. Populations of focus served by Case Managers include individuals and families who would benefit from enhanced care management and/or who are experiencing housing insecurity or homelessness; substance use disorder or mental health issues.

Health benefits, paid time off and retirement match available for full and part-time employees working over 24 hours per week. The salary range is $22 to $30 per hour depending on qualifications and experience.
