POSITION DESCRIPTION:
The Reentry Case Manager is a full-time grant-funded position administered through the Reentry Association of Western New York (RAWNY). RAWNY is a not-for-profit agency created as a reentry one-stop to provide services and support to individuals released from local, state, or federal correctional settings. RAWNY staff provide case management, referral services, and client support by addressing systemic barriers to reentry, enhancing public safety, and increasing successful reentry.
The ideal candidate must be client-centered and have knowledge and experience providing stabilization and support services for justice-involved clients. The focus of this position will primarily be on the client's medical, mental health, and substance use needs, ensuring connection to treatment. The candidate will collaborate with community providers connecting a population of diverse clients with a range of comprehensive services, embracing a holistic, individualized approach. The candidate must demonstrate a commitment to working as part of a multicultural setting and will communicate with empathy and understanding while demonstrating sound judgement and implementing best practices.
DUTIES AND RESPONSIBILITIES:
The candidate will be responsible for the following duties and responsibilities:
- Understand and acknowledge the core values and mission of the agency
- Create a positive and welcoming environment for clients, visitors and staff as a committed team member
- Conduct initial intakes, create client files, determine program eligibility and prioritize client needs
- Provide individual clients assistance with issues related to medical, mental health and substance use, benefits, employment, housing, childcare, healthcare, obtaining identification, and other identified barriers to reentry
- Be flexible and respond daily to clients with care coordination, referrals to in-house programs and community partners and follow-up to verify client engagement
- Input routine updates into the case management system and enter progress notes, in a timely and accurate manner consistent with agency regulations
- Engage in advocacy and build a robust positive professional relationship with community stakeholders, RAWNY partner agencies, and other governmental offices
- Attend collaborative partnership meetings as needed and remain current and knowledgeable regarding reentry resources within Monroe County and beyond
- Responsible for ensuring compliance with all confidentiality requirements including HIPPA and adhere to all agency policies, procedures and regulations
- Able to work flexible evening hours as needed for in house medical clinic
- Perform other duties as requested by a supervisor or the management team
SKILLS AND QUALIFICATIONS REQUIRED:
- Bachelor’s or Associate’s Degree with at least one (1) year case management experience in social work, criminal justice, psychology, counseling, or a closely related field. An equivalent combination of education and experience including lived experience, will be considered
- Experience working with formerly incarcerated clients, individual with mental health, substance abuse history and/or co-occurring disorders
- Maintain a clean driver’s license and provide proof as requested
- Understand the effects of trauma and provide trauma informed care
- Demonstrate compassion while working with clients to create support groups and develop prosocial recreational activities
- Proficiency in computer software such as Outlook, MS Word, Excel, Power Point, etc.
- Ability to accurately and timely enter client data into a case management system
- Must be highly organized and multi-task with refined interpersonal communication and problem-solving skills
- Adapt to a fast-paced environment, working both independently and as an integral member of the team
- Able to pass random drug screen as requested
- Familiar with community resources and be willing to interface with community partners and attend meetings and engage in outreach for clients
- Bilingual applicants are encouraged to apply
- Certification in Offender Workforce Development (OWDS), Moral Reconation Therapy (MRT), Thinking 4 Change (T4C), CBT, and/or CASAC are desirable but not required
- Willing to work a flexible schedule to provide coverage for weekend/evening groups
- Candidate can pass a clearance to enter the Monroe County Jail to meet with potential clients
BENEFITS INCLUDE:
- Full-time schedule working 35 hours weekly
- (Schedule: Monday - Thursday 8 am – 4 pm and Friday 8 am –1 pm)- Hours may need to be flexed to cover evening/weekend groups as needed
