Job Duties and Responsibilities:
- Conduct comprehensive assessments of clients' social, emotional, and psychological needs.
- Develop, implement, and evaluate care plans in collaboration with a multidisciplinary team.
- Provide counseling and support to clients and their families, addressing social and emotional concerns.
- Liaise with community resources and external agencies to coordinate additional support services.
- Advocate for clients' rights and needs within the facility and in the broader community.
- Participate in team meetings and contribute to the development of policies and procedures related to social work and client care.
- Bachelor's or Master's degree in Social Work from an accredited institution.
- Valid Licensed Social Worker (LSW) certification.
- Demonstrated experience in a healthcare setting, preferably in a care facility or similar environment.
- Strong interpersonal and communication skills, with the ability to work effectively with diverse clients and teams.
