Position Overview:
As a MDS Coordinator at Harwich, you will be responsible for managing the Minimum Data Set (MDS) assessment process, ensuring accurate and timely completion of all assessments for our residents. You will work closely with the interdisciplinary team to assess, plan, and document care while ensuring compliance with all state and federal regulations.

Position Overview:
As a MDS Coordinator at Harwich, you will be responsible for managing the Minimum Data Set (MDS) assessment process, ensuring accurate and timely completion of all assessments for our residents. You will work closely with the interdisciplinary team to assess, plan, and document care while ensuring compliance with all state and federal regulations.
Key Responsibilities:
- Conduct and coordinate MDS assessments for all residents in compliance with federal and state regulations.
- Complete the MDS assessments accurately and within required timeframes.
- Collaborate with physicians, nurses, social workers, and other team members to develop individualized care plans.
- Review and analyze resident data to ensure it accurately reflects the needs and services provided.
- Ensure compliance with Medicare, Medicaid, and state reimbursement guidelines.
- Maintain up-to-date knowledge of MDS processes, regulations, and best practices.
- Provide education and training to staff regarding MDS procedures and documentation.
- Coordinate care plan meetings and contribute to interdisciplinary team discussions.
- Assist with audits, surveys, and inspections to ensure compliance with all regulatory requirements.
- Current RN or LPN license (preferred) or equivalent healthcare experience.
- Previous experience with MDS assessments and the Resident Assessment Instrument (RAI) process.
- Strong knowledge of state and federal regulations, including Medicare and Medicaid guidelines.
- Excellent organizational, analytical, and communication skills.
