Responsibilities
- Assist guests with financial and legal matters, such as pension applications and referrals to lawyers or funeral homes.
- Provide discharge planning services, including arranging home care services, transfers to other facilities, and safe discharges.
- Offer counseling services and psychiatric medication management as needed.
- Maintain accurate and timely documentation in MDS care plans.
Qualifications
- Current social work licensure in the state.
- Bachelor's degree in social work required; master's degree preferred.
- One year of supervised social work experience in a healthcare setting is preferred.
