What are the responsibilities and job description for the Social Services position at Regency Healthcare and Rehab?
Regency Healthcare and Rehab is seeking a compassionate Director of Social Services, to serve our residents, in our fast-paced rehab facility. Social services are services provided by the facility’s staff to assist residents in maintaining or improving their ability to manage their everyday physical, mental and psychosocial needs.
A highly qualified candidate is an individual with:
- A bachelor’s degree in social work or a bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation counseling and psychology.
- One year of supervised social work experience in a health care setting working directly with individuals.
- Licensed in social work preferred, but not required.
Key Job Functions:
- The social worker, or social service designee, will complete an initial and quarterly assessment of each resident, identifying any need for social services of the resident. Any need for social services will be documented in the medical record under social services.
- The social worker, or social services designee, will pursue the provision of any identified need for social services of the resident. Attempts to meet the needs of the resident will be handled by the appropriate discipline(s). Services to meet the resident’s needs may include:
- Making arrangement or obtaining needed adaptive equipment, clothing and personal items.
- Working with residents, resident’s families and care plan teams on current goals, discharge planning and encouragement to participate in care planning.
- Informing residents and those they designate about available health care choices and their ramifications such as advance directives and code status.
- Making referrals and obtaining services from outside entities (e.g., talking books, absentee ballots, community wheelchair transportation).
- Assisting resident with financial and legal matters if appropriate.
- Discharge planning services
- Providing or arranging provision of needed counseling services.
- Through the assessment and care planning process, identifying and seeking ways to support residents’ individual needs
- Promoting actions by staff that maintain or enhance each resident’s dignity in full recognition of each resident’s individuality.
- Assisting residents to determine how they would like to make decisions about their health care and whether or not they would like anyone else to be involved in those decisions.
- Finding options that most meet the physical and emotional needs of each resident
- Providing alternatives by understanding and communicating to staff why residents act as they do, what they are attempting to communicate and what needs the staff must meet.
- Meeting the needs of residents who are grieving
- Fining options which most meet a resident’s physical and emotional needs
- The resident’s plan of care will reflect any ongoing medically-related social service needs and how these needs are being addressed.
- The social worker, or social service designee, will monitor the resident’s progress in improving physical, mental and psychosocial functioning.
- Preparing and sending death certificates and monthly death report to the state.
- Completing level ones if needed on admissions and following any required changes outlined by the OBRA office.
- Completing sections of MDS related to social services
- Attending resident care plans
- Serving as back-up for Admissions
- Visits to hospitals
- Marketing with Admissions/Therapy when needed
- Other duties as assigned.
