Home Health Nurse

We are currently seeking a compassionate LPN or RN to provide in-home care to an adult client with complex medical needs. Our patient requires specialized care, including G-tube feeding, oxygen therapy, medication administration, seizure disorders, and assistance with incontinence.

Key Responsibilities:

• Provide specialized in-home care for pediatric and elderly clients, including:
• G-tube feeding and monitoring.
• Monitoring of respiratory function.
• Administration of oxygen therapy.
• Assistance with incontinence and related care.
• Monitor patient vital signs, assess conditions, and adjust care plans as needed.
• Administer prescribed medications and treatments.
• Diabetes care
• Provide patient education and train family members on medical procedures and daily care routines.
• Maintain accurate patient records, documenting all treatments, assessments, and patient progress.
• Ensure a clean, safe, and comfortable environment for the patient.

Schedule:

• Afternoons 3:30pm - 7:30pm Monday - Friday and every other weekend Nashua, NH
• Afternoons 2:30pm - 8:30pm Monday - Friday Amherst, NH
• Overnights 12am - 7:30am Monday - Friday Brookline, NH
• Additional Hours Available: Opportunity to pick up extra shifts during the week if desired.

Qualifications:

• LPNs: Valid state LPN license.
• RNs: Valid state RN license.
• Previous experience with G-tube feeding, seizure disorders, and oxygen therapy is strongly preferred.
• Compassionate, patient, and professional demeanor with a strong commitment to quality care.
• Ability to work independently, with excellent critical thinking skills.
• Strong communication and interpersonal skills, with the ability to educate and communicate effectively with families.
• Must pass background check screening.
