Registered Nurse (RN) Case Manager
Job Type : Full-Time, Part-Time
Schedule : Day shift (Monday to Friday); weekends available if requested / preferred
Location : on the road in the Twin Cities area (Dakota & Ramsey County)
Job Description :
Regency Home HealthCare has immediate opportunities for compassionate people to join our team! The RN Case Manager provides a combination of direct care by performing hands-on nursing, advocating for, and collaborating with clients & their families, and keeping communication open with the entire healthcare team. Represent the company’s values while maintaining a professional working relationship with others. Perform a combination of hands-on care for medically complex clients on skilled nursing visits. Advocate for clients and assist them in achieving their goals and independence. Follow all guidelines and remain in compliance with documentation and policies.
Requirements / Qualifications :
- REQUIRED : Hold current RN license in the state of Minnesota
- REQUIRED : Hold current CPR certification
- Possess excellent leadership & communication skills as well as attention to detail
- Demonstrate consistent integrity, good judgment, and initiative
- Must have basic computer skills and the willingness to learn new EMR software
