SOCIAL SERVICES DESIGNEE
We are looking to have an energetic individual join our leadership team as our Social Services Designee! This is a full-time position.
General Purpose:
Responsible for the operation of the Social Services department, providing for each resident’s social, emotional and psychological needs and the continuing development of the resident’s full potential from admission to discharge.
Essential Job Functions:
• Develop a social history, social assessment and care plan which identifies pertinent problems and needs, realistic goals to be accomplished, and the specific action to be taken in resolution of the problems and/or needs upon admission of each new resident.
• Participate in the pre-admission screening of all potential residents and provide information to residents and families during admission.
• Orient the resident to the long-term care environment and facilitate adjustment upon placement.
- Acts as liaison between residents/families and the facility staff for concerns, questions, and in particular, personal needs
- Visits 1:1 with residents to assess psychosocial needs and to provide reassurance and support
- Completes a Patient Health Questionnaire (PHq-9) and Brief Interview for Mental Status (BIMs) per facility policy
• Document progress notes which relate to each resident's care plan when necessary.
- Communicates with residents/responsible parties about room changes and coordinates changes with facility staff
• May complete some MDS assessments.
• Assist the resident and resident's family in discharge and placement planning.
- Make referrals to appropriate home health agencies as requested by resident/responsible parties
- Assists with obtaining needed home health equipment
• Organize family groups to promote communications, education and support between family members, facility staff, and administration and provide counseling, as needed.
• Arrange to purchase needed items for residents and ensuring that residents have their needs adequately met.
• Assist in the education of the community regarding aging, resident rights, facility services and other related topics.
• Review and adhere to departmental budget.
• Participate in daily Interdisciplinary meetings and other community meetings including Care Plan meetings and Quality Improvement.
• Must be adaptable, flexible to changes, and able to prioritize and organize work efficiently to accomplish workload within time allotted.
• Maintain a positive attitude in relation to customer service program.
• Comply with established safety rules and regulations.
• Promptly reports accidents, incidents, and unsafe and hazardous conditions/equipment to supervisor.
• Understand, comply with, and promote all rules regarding resident's rights.
• Abide by the facility's policies, procedures and practices.
• Participate in and support Quality efforts throughout the facility.
• Attend meetings and mandatory in-services.
• Knowledge and adherence to safety/disaster preparedness plan.
• All other duties as needed.
Qualifications:
• Post Secondary Education with a degree in a related field.
• Minimum of two years in long-term care or acute care is preferred.
• Must have a minimum of 2 years of experience in a related social services field
• Must be able to relate professionally, positively, and cooperatively with residents, resident's families and employees.
• Must possess proficient computer skills, including email use.
• Must be capable of maintaining regular attendance.
• Must meet all local health regulations and pass post-employment physical exam if required. This requirement also includes drug screening, criminal background investigation, and reference inquiry.
