About the Role
The successful candidate will provide psychosocial support to patients and their families, working closely with our healthcare professionals to ensure comprehensive care. This role requires strong communication and interpersonal skills, as well as the ability to work in a fast-paced environment.
Responsibilities
- Provide assessments and develop individualized plans of care for patients and their families
- Collaborate with the healthcare team to ensure seamless care delivery
- Maintain accurate records and documentation
- Participate in multidisciplinary meetings to discuss patient progress and goals
Requirements
To be successful in this role, you will need:
- A Master's Degree in Social Work from a CSWE-accredited school
- Licensure as required by the state in which the hospice is located
- At least one year of experience as a Social Worker in a healthcare setting, preferably in hospice care
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment
