Job Description:
- As a Hospice RN/ RN Case Manager, you will be responsible for providing direct patient care services, managing patient clinical records, and participating in quality assessment and performance improvement programs.
- You will also serve as a primary liaison between physicians, patients/families, and the hospice team, empowering caregivers through education and promoting comfort, peace, and dignity for our patients.
Requirements:
- A minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology.
- Prior experience in hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing is strongly preferred.
