Position Overview : We are seeking a dedicated Hospice Patient Care Manager to elevate our patient care services. In this pivotal role, you will report directly to the Executive Director, Administrator, or Senior Patient Care Manager. Your core responsibilities will include :
- Managing and overseeing hospice program activities to ensure high-quality patient care delivery.
- Handling rescheduled, declined, missed, and reassigned visit requests, exercising clinical judgment to prioritize staffing needs.
- Responding empathetically to phone referrals and inquiries, triaging medical concerns within your scope of practice.
- Facilitating the Interdisciplinary Group (IDG) meetings, overseeing patient reviews, and coordinating relevant documentation while ensuring comprehensive communication among the care team.
