Key Responsibilities:
- Care Gap Management: Identify patients with outstanding care needs and facilitate gap closures through appointment scheduling and follow-ups.
- Patient Outreach & Engagement: Conduct outreach via phone calls, mail, and text messages to encourage patients to attend scheduled appointments.
- Data Verification & Reporting: Review patient lists, verify insurance attribution, and ensure accurate documentation in tracking systems.
- Collaboration & Coordination: Work closely with providers, nurses, and MCO representatives to support HEDIS measures and ER utilization initiatives.
- Administrative Support: Maintain accurate records, assist with scheduling, and support disease management programs.

Required Qualifications:
- High school diploma or specialized training/certification (LPN or Credentialed Medical Assistant preferred).
- 1-2 years in a healthcare-related role with knowledge of medical terminology.
- Strong communication, attention to detail, and ability to work independently with minimal supervision.
- Intermediate experience with Microsoft Office (Excel, Word, Outlook).
