Description

POSITION SUMMARY:
The Individualized Living Specialist (ILS) works in the Individualized/Supported Living Program which promotes and supports the life-style choices of adults with developmental disabilities as they live interdependently in the community. The ILS is responsible for meeting needs and providing documentation of consumer plans including Personal Support Plans (PSPs) and Individualized Family Service Plan (IFSPs) agreed to by the team of the individual; coordinating staffing and maintaining documentation of direct service staff; completing administration duties on behalf of the individuals cost plan; and maintaining confidential consumer files ensuring compliance of company policies and State contracts, laws and regulations. Supervise Direct Service Providers (DSPs) by instructing, resolving problems, providing feedback, scheduling and documenting meetings and contacts. The position reports to the ILS Program Manager/and Remote office Program Manager.
ESSENTIAL DUTIES & RESPONSIBILITIES:
1. Complete and or assist in intake visit activities with consumer.
2. Participate in transition services by coordinating with other provider agencies by act as billing/case manager for transitioning adolescents until they enter adult services.
3. Educate families about other services (CFC/CM/Voc Rehab).
4. Collect data (e.g., M. Smull/ Lifestyle/wellness/annual health care list/support information/personal finances) and compiles information on behalf of the individual for the PSP team and as documented on thePSP. Billed as part of Direct Service time.
5. Follow the PSP policy timelines, disseminates required information summaries to the Case Manager prior to PSP meeting.
6. Participate in PSP meetings. The position meets annually with consumer’s planning team; completes required sections of the PSP; develops action steps of outcomes pertaining to agency agreed upon services; develops emergency back-up plans. Billed as part of direct service time for individual.
7. Implements assigned responsibilities of the PSP/IFSP through contacts with the individual and direct care staff.
8. Develop service contracts and agreements; track cost plan utilization by tracking spending trends across categories, services and supports identified in the PSP; and submitting requests for changes in category allocations to the Case Manager for review and approval. On behalf of the individual, all tasks listed above that designate FO as service provider will be billed according to time needed to complete tasks.
9. Complete PSP quarterly progress reportson actions and outcomes that ILS is responsible for; submitting quarterly report to Case Manager; and submitting summaries of closed out Actions for the Annual PSP meeting. ILS Follows all designated timelines.
10. Supervise Direct Service Providers (DSPs) by providing input into selection and matching, providing instructions and training, resolving problems, providing performance feedback, and documenting meetings and contacts with DSPs. Regularly scheduled meetings with DSP will be an action plan in PSP.
11. Manage DSP time for assigned consumers by developing schedules for coverage, reviewing time sheets for accuracy, and ensuring services fit within consumer budgets through Celayix.
12. Document instruction provided to support staff. Health and Safety may be addressed on behalf of the individual as documented on PSP (i.e. housing needs) and billed by contracted agency service provider (FO).
13. Document contacts on behalf of the individual in program currently designated by agency to use in a timely manner.
14. Perform and bills as Direct Care Staff/DSP as identified in the PSP to ensure meeting contracted numbers in the cost plan.
15. Safeguard and maintain individual records to keep consumer files complete, confidential, and in order. This includes filing consumer documents in secure areas; adhering to the recommended organization of components of individual files; and obtaining and documenting Releases of Information agreements prior to sharing private consumer information with others.
16. Attend in-services, conferences, workshops, and required agency meetings as appropriate.
17. Communicate as necessary with other Family Outreach staff providing Supported Living.
18. Participate in review, revision, and/or addition to Family Outreach Policies and Procedures manual.
19. Assist with PR activities.
20. Participate in determining and implementing the annual objectives for Family Outreach.
21. Staff the after-hours on-call system on a rotating basis. See personnel policies.
22. Follow the Incident Reporting policy.
23. Participate in the on-call rotation involving taking the on-call phone for designated periods, immediately responding to calls and emergencies (either responding in person or arranging staff responses), and answering or coordinating answers to any questions or issues that may arise.
ADDITIONAL DUTIES:
1. Perform a variety of other professional and administrative work as assigned by the supervisor.
2. Coordinate special projects and events.
3. Attending training as required.
4. Provide backup and coverage for other Family Outreach staff.
5. Other duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES
 Knowledge of the principles and practices of social work, developmental disability intervention, adult education and learning methods.
 Knowledge of community resources.
 Knowledge of state and federal policies, legislation, rules and regulations.
 Skills in technical writing and record-keeping methods and techniques.
 Skills in maintaining public relations by communicating effectively with community professionals.
 Skills in the operation of general office equipment including computers and typical business software applications.
 Skills paying attention to detail in order to match providers with consumers, accurately processing forms, and organize and prioritize work.
 Ability to work effectively for and with individuals with intellectual and behavioral differences.
 Ability to effectively support consumer's to develop a community network in their community.
 Ability to research and assess consumer needs and design informational and educational materials, methods and learning activities.
 Ability to conduct training sessions.
 Ability to follow verbal or written instructions and complete work assignments without continual direction.
 Ability to work flexible hours, including evenings, weekends, overnights, and holidays.
 Ability to act independently and function as part of an overall team.
 Ability to maintain a positive image of the agency.

Qualifications


MINIMUM QUALIFICATIONS:
 Bachelor’s degree from an accredited college or university required.
 Valid Montana driver’s license or ability to obtain in one month of hire.
 Access to reliable vehicle.
 Successfully complete a background check.
PREFFERED QUALIFICATION:
 Bachelor’s degree in sociology, social work, psychology or education will be given preference over candidates with a degree in another field.
 Three (3) years related working experience including service coordination experience.
 Skill in communicating effectively verbally and in writing.
 Ability and willingness to align behavior with the values of the organization.
INDEPENDENCE OF ACTION
The position is supervised by the Supported Living Program Manager and is directed by the Program Manager on individual issues to provide technical assistance. This position is responsible for promoting and supporting the life-style choices of adults with developmental disabilities while. Work is performed according to Policies and Procedures established by the Family Outreach Board of Directors and under supervision of the Executive Director.

WORKING CONDITIONS
Work is performed in a normal office environment and in client homes. The position may involve overtime and weekend work during peak workloads. (see personnel policies) The position involves physical demands associated with travel by auto, working on a computer, communicating over the phone and in person, and light lifting and filing.
