CLASSIFICATION TITLE: Case Manager (Children)
GENERAL STATEMENT OF RESPONSBILITIES: Seeking an individual to join our collaborative team in supporting youth and families impacted by intellectual or developmental disability and/or mental health diagnoses and emotional and behavioral challenges. The case manager has the responsibility for providing the coordination and monitoring of services to individuals under 18 with a primary diagnosis of intellectual or developmental disability, a mental health disorder, emotional disturbance, or at risk of either. This includes assessing service needs, developing plans for access to services, acting as liaison with consumer, family, service providers and programs, ongoing monitoring of service needs, problem-solving and advocacy.
QUALIFICATIONS: A Bachelor’s degree and at least two years of experience in the human services field or a combination of equivalent education and experience is required. Preferred candidates will meet requirements as QMHP-C (Qualified Mental Health Professional – Child).
