What are the responsibilities and job description for the Partners in Health Coordinator position at Regional East Texas Food Bank?
POSITION SUMMARY
The Partners in Health Program Coordinator is responsible for implementing and expanding our healthcare-food bank partnership, Partners in Health (PIH). The coordinator serves as a liaison between the food bank and health care organizations, providing training and technical support to partner clinics.
POSITION GOALS
- Coordinate and administratively support leadership meetings and facilitate team/workgroup meetings.
- Complete and maintain IRB submission (with supervision by Nutrition Education Program Manager).
- Work with external stakeholders to identify and onboard potential participating clinics.
- Initiate and provide ongoing communication with external partners and participating clinics to achieve the goals of the program.
- Recruit new clinic partners to grow the program.
- Establish working relationships with healthcare providers and clinic staff to ensure the sustainability of the program and expand the pathway for patient enrollment/use of the PIH platform.
- Develop and demonstrate educational presentations on food insecurity for clinic staff.
- Facilitate clinic flow training for participating clinics.
- Create workflow charts for each participating clinic.
- Provide ongoing technical assistance to participating clinics.
- Communicate with healthcare partners to discuss project goals and utilization of the platform.
- Ensures clinic sites have needed program materials for patient enrollment.
- Assist with data collection including coding and recording of data.
- Draft monthly data reports and share with healthcare partners.
- Work with evaluation team on program evaluation for program improvement and external sharing.
- Assist with presentations and written program documents.
- Manage Qualtrics platform (and connected services) for the use of the referral form and other programmatic needs.
- Complete nutrition education component of the Partners in Health Program.
- All other duties as assigned.
WORKING CONDITIONS
This position works in a remote environment but also requires travel approximately 25% of the time to various healthcare organizations and off site locations. This position requires some physical activity such as lifting and carrying up to 20 pounds.
QUALIFICATIONS
- Education and Experience
- Bachelors degree in nutrition, health, or relevant field
- 2 years experience in a nonprofit organization or clinical setting preferred

- Technologically proficient. Intermediate skill level in Microsoft Office programs and ability to learn new software systems
