Regional Hospice is looking for a FULL TIME RN Case Manager for our Home Hospice patients.
We are looking to hire an experienced RN (preferably in hospice) to work full time, 40 hours, Monday-Friday, 8:30 am – 5 pm. The Case Manager is responsible for the coordination and management of all patient care and other services for patients and their families. The Case Manager works with interdisciplinary team and the patient’s primary care physician to provide quality pain and symptom management, as well as support to the patient and family.
Qualifications
- Registered Nurse with a current license in the State of Connecticut REQUIRED.
- Minimum of two (2) years’ experience as a staff nurse in a hospital, plus additional experience in hospice or home health care agency is REQUIRED.
- Advanced credentials/certification in hospice care (CHPN) preferred.
- Transportation required, including proof of auto insurance and a clear driving history.
- Excellent verbal, written and computer skills – ability to communicate effectively with patients, families, physicians, interdisciplinary team members and others.
- Bachelor of Science in Nursing degree desired.
The RN Case Manager must possess good communication and organizational skills and be able to work in a variety of environments. The RN Case Manager must present a professional attitude and appearance at all times while maintaining strict confidentiality of patient and agency information.
