What are the responsibilities and job description for the Registered Nurse (Home Care) position at REGIONAL MEDICAL CENTER?
Sign on Bonus:
- Up to $10k bonus with 2 year commitment, home care experience required
- $1500 at 90 days and $1500 at 1 year if no home care experience
This is a full-time, salaried (exempt) position that is scheduled 32 hours on call; Monday - Thursday. No scheduled hours or call on Friday, Saturday or Sunday. Rotating holidays.
This position performs direct patient nursing care to patients of the Home Care department in accordance with hospital and departmental standards.
- Provides direct nursing care to Home Care clients
- Support the patient’s and family’s unique spiritual and cultural beliefs.
- Acquires knowledge and assesses condition of clients through interview, history, physical, referral source and significant others
- Observes, records, reports any changes in any signs, symptoms and condition of the client
- Assess nursing care and home care aide needs of the client
- Assists the physician in establishing a plan of care for the client
- Assumes responsibility for carrying out the plan of care
- Provides education to the client and caregivers, as appropriate
- Assists in recognition and referral of the client to ancillary services, including local/community resources
- Promotes family well-being and holistic client care
- Participates in interdisciplinary team functions
- Communicates needs for inservice education
- Assists with orientation of new employees
- Assists with infection control problem identification and documentation
- Updates client care plan and progress notes with each visit
- Assists with scheduling of client visit
- Performs collection of lab specimens
- Assists in checking supplies and communicates inventory needs
- Supervises home care aides, in-home health providers, Home Care LPN and CDAC providers, as needed
- Communicates admissions and discharges via time study
- Assumes responsibility for client admission/discharge and recertification paperwork
- Follows agency direction for completion of comprehensive assessments at required time points including OASIS assessments
- Accurately completes time study, including both sides
