Job Summary
Service Coordinator receives referral for early intervention program; develops, maintains, and processes the permanent early intervention case record; coordinates evaluation and provision of services for children ages birth to three with developmental delays or disabilities; provides families with information about the early intervention service system.

Responsibilities and Duties
Know, understand, and follow the Health Insurance Portability and Accountability Act (HIPPA) and Family Educational Rights to Privacy Act (FERPA).Case Management
Conduct and complete intake with family
Coordinate evaluation of child’s development to determine program eligibility and need for services
Work with team consisting of family and providers to develop Individual Family Service Plan (IFSP)
Monitor implementation of plan, and work with team to update as necessary
Maintain complete, organized case records and ensure that all deadlines are met
Ensure that transition planning, case transfer, and case closure occur consistent with state and federal requirements.Perform other
- Qualifications and Skills
Bachelor’s Degree or higher in Human Services, behavioral science, social science or health related field; a current license as a Registered nurse.
Case management experience strongly recommended
Excellent communication skills & strong interpersonal skills
Familiarity with community resources and other area early childhood programs
