Outreach Coordinator - Perinatal Telehealt

A Brief Overview
As an integral member of the Perinatal Telehealth Program (Stork 901), the Outreach Coordinator is Responsible for achieving the program's growth targets.
What you will do
- 1. Acting as liaison between the Regional One Health System and providers in West TN as appropriate, to coordinate high risk obstetrical care for referred patients.
- 2. Increasing awareness of the Perinatal Telehealth Program among the targeted Obstetrics and Family Practice providers in the area and influencing their referral decisions by articulating and promoting the program’s value proposition.
- 3. Establishes and develops productive partnerships with both new and existing providers in selected geographic areas
- 4. Builds the data base of patients including detailed profiles and tracks and manages interactions including all care coordination efforts.
- 5. Develops visitation schedules to ensure consistent personal visibility and presence at provider locations.
- 6. Effectively communicates the Perinatal Telehealth Program’s (Stork 901) value proposition to all appropriate personnel at provider location.
- 7. Conducts follow-up meetings with providers to gain feedback on performance and maintain customer satisfaction.
- 8. Actively engages providers by using a mix of appropriate and cost effective communication channels, such as emails, social media and web based interactions.
- 9. Accesses relevant data bases and reporting in order to monitor and analyze performance. Prepares and submits monthly activity tracking metrics to the Practice Manager.
- 10. Serves as liaison between Regional One Health System and clinical operations resources of local and regional office sites as appropriate to coordinate patients initial high risk obstetrical treatment.
- 11. Works in collaboration with Regional One Health’s Maternal Fetal Medicine’s medical staff to obtain patient’s status in order to communicate progress to the referring provider.
- 12. Ensures through internal monitoring and advocacy, that all agreed upon data, reports and other deliverables for the patient are received, as appropriate.
- 13. As the program’s relationship leader, communicates with the Stork 901 case manager, as necessary, to ensure rapid service issue resolution.
- 14. Increases the awareness of the Perinatal Telehealth Program (Stork 901) among healthcare providers in the designated market areas.
- 15. In collaboration with marketing, develops marketing strategies and materials to promote the Perinatal Telehealth Program (Stork 901).
- 16. Conducts research, identifies relevant industry, business and professional organizations and actively participates in functions.
- 17. Coordinates special events and projects as required.
- 18. Represents the Perinatal Telehealth Program (Stork 901) at key industry and community meetings.
Qualifications
- Bachelor's Degree in Business, Healthcare Administration, or related area Required
- Minimum 3 years experience in business development/sales with a minimum of 1 year in healthcare. Required
- Women's healthcare experience Preferred
