A Brief Overview
Provides clinical services which include drug information, drug therapy review and evaluation regarding medications, drugs and other pharmaceuticals to medical/hospital staff. Performs duties on an as needed basis as defined by the float pool and/or PRN guidelines.

What you will do

  • Reviews patient medical and pharmaceutical records and monitors for efficient drug therapy, drug interaction, adverse drug reactions, contraindication, allergies, and sensitivities.
  • Communicates directly with both physicians and nurses regarding questions concerning patient drug therapy. * Provides pharmacotherapy consults to staff and consultative services to patients. * Evaluates, monitors and recommends patient's drug therapy to support and provide quality services and maintain a therapeutic and cost efficient drug regimen. * Recommends alternate drug therapy or regimens, as necessary. * Provides kinetic consultations within specified time frames. * Monitors all patient drug therapy for cost effectiveness and communicates all possible cost saving recommendations to patient physician/provider(s).
  • Monitors all departmental patient-related financial records that are maintained with the assigned work area to assure accuracy and timeliness of processing; assists supportive personnel with processing when appropriate.
  • Develops and demonstrates commitment to the self-directed development of clinical pharmacy practice skills under guidelines of the departmental staff development programs through identification of personal education goals and objectives. * Publishes/Participates/Submits research articles, grant proposal, poster or verbal presentations. * Coordinates Clinical Pharmacy in-services for attending physicians, nurse staff, pharmacists, ancillary departments or patient groups. * Participates in supervision and professional development of pharmacy residents.
  • Maintains current knowledge of State and Federal regulations and statutes regarding the practice of pharmacy and the distribution of approved drugs, controlled substances and investigational agents. * Participates in P & T activities. * Prepares formulary drug evaluations and completes medication usage evaluation reviews. * Documents monthly interventions. * Documents monthly adverse drug reactions.
  • Performs functions and activities of staff pharmacist as needed and in accordance with professional standards and practices and established policies and procedures.
  • Operates computerized data systems. * Interprets and performs order entry functions in accordance with departmental policies and procedures. * Utilizes computer software applications to obtain drug information. * Operates computer and related equipment to enter and retrieve patient information. * Update patient information as required.
  • Works effectively and serves as intermediary between patient care units and pharmacy; answers calls and provides information on matters related to Pharmacy Services. * Interacts and communicates in a customer responsive and courteous manner, in compliance with iRESPECT standards and promoting patient/customer satisfaction. * Provides information on drugs, pharmaceutical to patients and medical/hospital staff in a professional manner. * Participates in drug research projects. * Works collaboratively with medical/hospital staff to coordinate and implement activities. * Provides in-services for staff and other departments. * Facilitates specialty pharmacy activities such as prior authorizations.
  • Provides instruction, assigns and supervises other support/ancillary personnel assigned to unit to provide clinical assistance as needed. Assists in the orientation and oversee the supervision of new staff members and students when they provide patient care, treatment, and services as part of their training.
  • Completes required continuous training and education, including department specific requirements. * Attends staff meetings, all mandatory in-service, and additional continuing education as required. Maintains membership and participates in professional organizations. * Contributes to the annual review of unit-based scope statement and determination of important aspects of care for clinical review. * Assists in the development of indicators, thresholds, study methods, and data collection as assigned. * Responds to problems/opportunities to improve care. * Supports and is involved in the organization’s Performance Improvement initiatives.
  • Maintains a clean, orderly, safe environment to minimize risk to patients and staff by complying with all safety/risk management policies and procedures. Adheres to established policies, compliance/quality improvement, and safety, environment and infection Prevention standards. * Practices aseptic techniques according to policy and procedures. * Assists in care and maintenance of equipment and effective utilization of supplies. * Cleans and maintains all pharmacy related equipment. * Reports malfunction of equipment to supervisor.
  • Obtains and maintains current certification required by the department to ensure abilities to deliver specialized services, such as Basic Cardiac Life Support, Mass Casualty Plan, etc.
  • Responsibilities may include budgeting, program evaluation, annual review, etc.).
  • Provides assessment, education and follow-up instruction for specified health programs (i.e., Diabetes self-management, etc.). * Participates in coordinating and developing new clinical services to improve quality of care provided. * Assists in development of continuing and education and quality improvement programs to optimize patient care
  • Prepares and maintains required records, reports and files for administrative, operational and regulatory compliance purposes. * Compiles and prepares various reports and data, collates and assembles reports and documents as required. * Generates statistics, computations, tables, or charts as required. * Utilizes analytic skills and statistical analysis to aggregate data to assist with the identification of potential problems and patterns, trends, establish priorities and recommend improvement activities.
  • Demonstrates knowledge technical, interpersonal and critical thinking skills to provide care, maintain clinical competency and meet age-specific patient needs within pharmacy areas. Interprets and monitors appropriate drug therapies resulting from age specific groups. This includes the neonate, infant, adolescent, adult, and/or geriatric patient population as specialty assignment dictates.

Qualifications
  • Pharm D from an accredited school of Pharmacy and the successful completion of an approved Pharmacy Resident Program. Required and
  • Qualified by education, training or experience to work with the neonate, infant, adolescent, adult, and/or geriatric patient population as specialty assignment dictates. Required
  • PHARMD - Licensed Pharmacist Tennessee Licensure as a Pharmacist. Upon Hire Required
  • Additional certification may be required based on area of assignment. Upon Hire Required
  • Experience in patient care as obtained through completion of an approved pharmacy program. Required
