The Community Health Worker (CHW) serves as a consistent care coordinator throughout the continuum of care for patients and families throughout their healthcare journey. The CHW helps patients communicate with their healthcare providers to ensure they receive the information needed to make decisions about their health and social needs. The incumbent coordinates communication with providers and other healthcare team members on behalf of the patient. The CHW promotes, maintains and improves population health by fostering connections that bridge systems of health, social services, community based organizations and individuals by addressing social drivers of health and improving psychosocial well-being.

What you will do
- Creates a seamless flow through the health system to facilitate the patient’s care.
- Facilitates improved health care access and quality for underserved populations through advocacy and care coordination.
- Organizes schedules and manages appointments for patients to ensure they receive services in a timely manner.
- Facilitates communication between the patient, family members, and healthcare providers to ensure patient satisfaction and quality of care.
- Manages health care records
- Informs the patient on financial aspects of care and linking patient to financial resources as needed.
- Proactively manages relationships with patients.
- Facilitates interpretive language services when needed.
- Manages follow up visits and ensuring continuation of care and linkage to community resources.
- Works collaboratively with all members of the healthcare team to meet the patient needs.
- Identifies and communicates any social challenges faced by the patient.
- Maintains visibility and accessibility to assigned patient base.
- Effectively manages time, workload and resources.
- Performs other job functions as assigned or requested.
- Uses approved screening tool to document SDOH and facilitates referrals to community agencies.
Qualifications
- High School Diploma Or equivalent Required
- Associate's Degree focus in social work, psychology, or other human service fields. Preferred
- BCLS - Basic Life Support (BLS certification will be obtained as part of onboarding if not already certified). Upon Hire Required and
- Additional training/certification may be required.
- Minimum 1 year experience Working in a community-based, multi-cultural setting. Required or
- Combination of health/social services job experience, lived experience and volunteer experience may be substituted Required
