About the Company - Regional One Health is currently seeking a Pharmacist to work PRN at our South Third Primary Care location.
About the Role - Provides professional pharmacy services. Reviews, interprets, monitors and dispenses medications, drugs and other pharmaceutical as prescribed by physician and according to professional standards and practices. Performs duties on an as needed basis as defined by the float pool and/or PRN guidelines.
Responsibilities -
- Prepares and dispenses medications as prescribed and ordered by physician/provider, including the calculation and preparation of solutions for oral, external, intramuscular and intravenous administration.
- Comply with all applicable regulations and standards.
- Receives physician/provider orders, preparing prescriptions and refill requests and intervenes when appropriate for clarification or correction.
- Interprets and evaluates medication orders for contraindications, incompatibilities, allergies, and dosing.
- Ensures patient profile is complete and adjusted accordingly for height, weight, current and past medical history, clinical laboratory results and concurrent medications (including over-the-counter and/or home medications) and to reflect provider’s desire to continue or discontinue said drug.
- Checks dosage for appropriateness for neonatal, pediatric, adult and geriatric patients.
- Accurately supplies medication to patients. Select, compounds and dispenses drugs and other pharmaceutical products. Verify accuracy of filled medication orders before dispensing.
- Performs the preparation of drug labels and the affixing of labels to the proper containers; ensures the inclusion of appropriate directions for the timing, methods and precautions to be observed in taking the medication; personally instructs patients in the use of medications when indicated.
- Prepares sterile and non-sterile drug products.
- Orders routine and special medications.
- Prepares and maintains records and other tasks normally associated with the operation of a Pharmacy in a clinical care context.
- Ensures accurate documentation in performing and processing all orders/supplies to capture all charges and other relevant information.
- Utilizes time in most effective and productive manner to ensure orders are delivered within established timeframes.
Qualifications - Graduate of an accredited school of Pharmacy Preferred and Master's Degree M.S. Preferred and Pharm D Preferred and Qualified by education, training or experience to work with the neonate, infant, adolescent, adult, and/or geriatric patient population as specialty assignment dictates. Required PHARMD - Licensed Pharmacist Tennessee Licensure as a Pharmacist. Upon Hire Required Additional certification may be required based on assignment. Upon Hire Required Experience in patient care as obtained through completion of an approved pharmacy program. Required
