We are seeking a highly motivated and compassionate Medical Social Services Representative or Social Worker to join our team. In this role, you will be responsible for providing emotional support and counseling to patients and their families, as well as coordinating with healthcare professionals to ensure the best possible care for our patients. Your main goal will be to improve the quality of life for our patients and their families during their time of need.

Medical Social Services Representative Minimum Qualifications:
- Bachelor's Degree in human services related area
- 2 years in human service area required
Social Worker BSW Minimum Qualifications:
- Bachelor's degree in Social Work
- Certified Social Worker through the Department of Health, State of Nebraska
Preferred Qualifications:
- Knowledge of community resources and support services
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Experience working with patients with chronic illnesses
Responsibilities:
- Provide emotional support and counseling to patients and their families
- Coordinate with healthcare professionals to ensure the best possible care for our patients
- Assist patients and their families in accessing community resources and support services
- Maintain accurate and up-to-date patient records
- Participate in interdisciplinary team meetings to discuss patient care plans
