Revita Life, an Axia Womens Health care center, is seeking an experienced and dedicated Nurse Practitioner to join our team that specializes in hormone replacement therapy. The ideal candidate will have a background in BHRT, exceptional clinical skills, and a passion for improving patients quality of life.
- Location : Sewell, NJ (Gloucester County)
- 3 Providers : 1 Physician, 2 NPs
- Monday-Friday hours - No Weekends!

The Nurse Practitioner performs physical examinations, confers with patients in person and by phone / electronically, collaborates with others in the department to ensure a smooth flow of patient care consistent with established quality indicators and participates in the overall coordination of patient care. Other job duties include but are not limited to :
Practices under the standard procedure established for the Nurse Practitioner role consistent with Company policy and in accordance with all legal requirements.Conducts comprehensive patient assessments, including medical history, physician examinations, and diagnostic evaluations consistent with department protocol.Performs assessment / data collection in an on-going systematic manner, focusing on physiologic, psychosocial, cultural, spiritual, and cognitive status, patient / family education, family involvement, and patient advocacy.Evaluates and care for patients, both men and women, for bio-identical hormone replacement therapy.Develops and implements individualized hormone replacement therapy plans.Performs pellet insertions.Consults with supervising physician or other clinicians as dictated by patient need and assessment findings.Participates with physicians and other clinicians in the department to assure effective patient care consistent with quality indicators for patients cared for in the designated service.Monitors and adjusts treatment plans based on patient progress and laboratory results under established protocols.Establishes effective working relationships with members of the health care team, patients, and families.Demonstrates knowledge of and applies safety principles as identified by the practice.Performs efficiently in emergency situations following established protocols, remaining calm, informing appropriate persons, and documenting events.Demonstrates knowledge of legal issues, including patient confidentiality and risk management in all aspects of patient care and unit functioning.Demonstrates a service philosophy in all interactions with patients, families, and all members of the health care team.Demonstrates proficiency in the use of computers and telecommunication modalities to document, track and convey information.Stays current with the latest research and advancements in hormone replacement therapy.Assists with daily patient phone triage.Qualifications :
Active, unrestricted New Jersey APN license, NJ CDS, and DEA.Board Certification as a WHNP, FNP, or ANP.Previous BHRT experience highly preferred.Strong understanding of endocrine system functions and hormone therapies and weight loss management.Eligible to participate in state programs including but not limited to Medicare and Medicaid.Benefits Summary :
