Job Title: Outpatient Therapist - LCSW, LMSW, LPC, or PLPC
Supervised by: Program Director
Job Summary: Implements the patient care as directed by the treatment team, responsible for conducting individual group therapies, participating in treatment planning, assisting patients and family members with psychosocial and financial issues and discharge planning.
Qualifications:
- A minimum of a degree in social work (LMSW) or Masters in Counseling (PLPC) from an accredited institution
- At least one year clinical experience in a psychiatric setting is required
- LCSW or LPC preferred.
Description of activities or skills:
- Coordinates the provision of social services to patients, families, and significant others to enable them to deal with the impact of illness on individual family functioning and to achieve maximum benefits from health care services.
- Assists in the development and is involved in the implementation of programs to assist patients and their families with the social and psychological factors of illness.
- Engages in individual or group counseling sessions pertaining to care and adjustment.
- Arranges for discharge and aftercare planning
- Coordinates the social services discharge and home care planning process to facilitate the continuity of care and the provision of effective patient and family support services.
- Coordinates the provision of information and guidance to patients, their families and other hospital departments regarding psychosocial aspects of patient’s post-discharge treatment.
- Coordinates the referral of patients and families to appropriate community, social service, home health services, extended care and rehabilitation facilities and organizations.
- Assists patients, families and significant others with financial issues and securing appropriate financial aid when indicated
- Coordinates, plans and presents in-service education for other personnel and outside groups as requested.
- Performs other duties assigned.
