This is a full-time on-site role for a Social Worker at REGO PARK HEALTH CARE in Queens, NY. The Social Worker will be responsible for providing counseling, support, and advocacy to patients and their families. They will also collaborate with healthcare professionals to develop and implement care plans, provide crisis intervention, and support patients in navigating healthcare resources.
Qualifications
- Strong communication and interpersonal skills
- Ability to provide counseling and support to individuals and families, manage resident/family grievances
- Experience in crisis intervention and resource navigation, advance directives, discharge planning and documentation
- Knowledge of healthcare systems and community resources
- Bachelor's or Master's degree in Social Work or related field
- Knowledge of PCC
- one year Nursing Home experience required
Salary : $75 - $85
