Primary Job Responsibilities:
- Conduct Patient Examinations:Perform thorough examinations, obtain medical histories, and assess the health status of patients
- Diagnostic and Therapeutic Services:Order, perform, and interpret diagnostic tests; prescribe medications and manage treatment plans
- Consultation and Referrals:Consult with physicians on complex cases and coordinate appropriate specialty referrals
- Patient and Family Counseling:Provide guidance on preventive health care and manage patient and family education to enhance understanding of treatment plans
- Triage and Patient Communication:Handle patient calls, evaluate concerns, and triage effectively to ensure timely medical responses
- Documentation and Reporting:Maintain comprehensive medical records and develop status reports to track patient progress
- Interdisciplinary Collaboration:Actively participate in interdisciplinary team meetings to discuss patient care, treatment modifications, and progress
- Facility Metrics Meetings:Engage in monthly meetings to discuss key facility metrics, including rehospitalization rates and acute care transfer rates, to improve facility outcomes and patient care

- Active Physician Assistant or Nurse Practitioner License (Required)
- In person with some remote documentation flexibility
- Minimum two years of experience as a PA/NP, preferably in an acute, subacute, or rehabilitation setting
